TC CSW

The CSW is responsible for providing intensive case management services, developing treatment/service plans, facilitating groups and helping to support clients in a residential setting for women leaving incarceration. Pavilions is Residential Community for women on probation/parole to receive supportive services as they transition into the community. This role functions in conjunction with a team for effective service delivery. All clients have been previously incarcerated and have co-occurring mental health diagnoses with drug/alcohol dependency. Case management services include crisis intervention, relapse prevention support and emergency services, ongoing psychosocial assessment, family support services, and vocational support.
